TURN AROUND TIMES

Our in-house Production Time will start on the next business day for all devices arriving after 10:30 a.m., not including weekends and holidays.  Our standard in house fabrication time is 5-7 days for our standard products , non standard products and High volume times of year do change fabrication thru put. If you have a really hard need by date please contact us prior to shipping. Should you need a device completed in a shorter time period, please call the PHOENIX HABILITATION TECHNOLOGIES office at 847-752-5728 prior to shipping. There will be an Expedited or Rush fee for services that need a faster turnaround time.  Less than 3 days (not including arrival day) is classified as “Expedited Turnaround” - there is a $50 upcharge per device and $75 for a pair. Same day fabrication (made the day it is received , ships the next day) is classified as Rush service - there is a $100 upcharge per device and $150 upcharge for a pair.

PRODUCT WARRANTY

 

PHOENIX HABILITATION TECHNOLOGIES hereby agrees that the   orthotic device produced by PHT ("Device") will be made to the written specifications provided by the customer and will be free from defects in materials and workmanship during the Warranty Period (as defined below). All defective components of the Device produced or manufactured by PHT will, at PHT’ option, be either repaired or replaced at no charge to the customer should they become defective or fail during the 90 days from the date of receipt of the Device to the customer ("Warranty Period"). You must notify PHT in writing or by email before expiration of the Warranty Period of a claim under this Warranty at PHOENIXHABTECH@GMAIL.COM Your failure to do so will result in this Warranty expiring at the end of the Warranty Period.

NEED TO KNOWS

 

We require the use of all PHT forms, habitual (more than 3 times) use of non PHT forms or incomplete forms will incur a consult fee of $40 to convert or finalize the order form in order to maintain the warranty.

 

PHT does not accept plaster casts, At this time we only accept fiberglass casts for manufacture. Please remove all interface material from cast at time of casting. Failure to remove interface material results in a $10 upcharge per cast

 

PHT reserves the right to refuse to fabricate any devices not in accordance with its procedures.

 

Cast Quality policy – cast quality is extremely important in our ability to provide you with the best outcome for your patient.  PHT reserves the right to refuse to manufacture a device from a sub-par cast. If this occurs PHT will notify the customer to request a new cast, if customer directs us to use said cast see "Sub-Par" cast below.

Casts must depict the patient’s anatomy for the device requested and of structural integrity to be adjusted if required. Casts that need to be reinforced due to material used or more than 2 alignment cuts (alignment planes) will incur a reinforcement up-charge. Casts requiring more than 3 cuts (alignment planes) are considered “sub-par” and the customer will be notified. If sub-par cast is directed to us to be used there maybe an additional modification charges added  per cast at PHT’ discretion as well as warranty implications  dictated by PHT.

Cast Height – due to the type of manufacture we utilize we require that the cast be 1” higher than desired finish height. We will only increase height up to 2” with an up-charge

Our finished height tolerance is + - ¼”

 

Special Order Components or Transfers: All non stock components can incur up to a 30% up-charge and up to 5 days or more delay in manufacture times.

PLEASE CONTACT US PRIOR TO CAST SHIPMENT TO DISCUSS ANY SPECIAL COMPONENTS OR TRANSFERS SO THAT THERE ARE NO DELAYS IN PRODUCTION.

NOTE:

 THERE ARE SOME COMPONENTS WE DO NOT DO: EXAMPLE = NYU TYPE FOOTPLATE, GAFFNEY JOINTS, ECT, RULE OF THUMB: IF IT DOESN'T HAVE A MOLDING DUMMY WE MOST LIKELY DON'T DO IT.

Over-sized cast: Any cast which has a circumference of 15” or greater and or 14” or greater in height will incur a size/material up-charge
